Adult Volunteer Evaluation/Check-In

I’ve almost been here at Mt. Hebron UMC for a year.  The other day I decided it was time for a check-up.  I searched, I tweeted, and used a previous form that I made and collaborated.  The end result was this evaluation for my volunteer leadership.  Take a look.  Let me know what you would add or subtract from it.  Also let me know if you end up using it for your group.
